Day shift, weekdays only from 8:30 AM to 5:00 PM. No weekends or call required.

Job Description

The Physical Therapist will provide home health services to patients, focusing on individualized care plans and rehabilitation techniques. Skills in documentation, patient assessment, and management are essential for success in this role.

Responsibilities

• Conduct thorough assessments and evaluations of patients
• Develop and implement personalized care plans
• Document patient progress and update plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Educate patients and families on home exercise programs and mobility techniques

Required Experience / Certifications / Licensure

• 1 year of Home Health experience required
• BLS certification required
• Current West Virginia State License or compact license accepted
• Experience with OASIS documentation is necessary
