Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development of the lifespan and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed by adult and geriatric patient group.

Responsibilities

  • Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team.
  • Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
  • Recommends revisions to the plan of care based on the evaluation of resident outcomes and indicates the need for referrals.
  • Maintain current knowledge and competency in Nursing Practice; participate in the mentoring/orientation of staff.
  • Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

  • New York LPN Licensure
  • BLS certification (must be through AHA/RQI)
  • 1 year of experience serving in an LPN capacity at a long-term care facility

Schedule

MUST be able to start full-shift orientation on a Tuesday or Thursday. EOW and holiday work is required.
