LPN - Full Time Days/Evenings Rotation

Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The LPN will provide direct and indirect skilled nursing care under the supervision of a Registered Nurse while maintaining a collaborative relationship with other nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of developmentally appropriate and culturally relevant care plans involving the resident/family and healthcare team.
• Recommend revisions to care plans based on evaluations of resident outcomes and indicate the need for referrals.
• Maintain current knowledge and competency in Nursing Practice and participate in mentoring/orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification through AHA or RQI
• Minimum of one year of experience as an LPN in a Long-Term Care (LTC) facility

Schedule

Must be able to start on a Tuesday or Thursday for full-shift orientation with a rotation of 5x8 hour shifts between days and evenings.
