Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care to assigned residents, ensuring the delivery of age-appropriate care for adult and geriatric populations. The LPN will work under the supervision of a Registered Nurse and will maintain collaborative relationships with other nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the Registered Nurse and interdisciplinary team members.
• Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and interventions for desired outcomes.
• Recommend revisions to care plans based on resident outcomes and facilitate necessary referrals.
• Participate in mentoring and orientation of new staff while considering safety, cost, and impact in care planning.
• Maintain current knowledge and competency in nursing practice and demonstrate leadership through professional practice.
• Handle occupational exposure to bloodborne pathogens and perform tasks involving potential contact with blood and body fluids.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS (AHA or RQI only)
• Minimum of 1 year of experience serving in an LPN capacity in a Long-Term Care facility
