Job Description

The Registered Nurse provides professional and comprehensive nursing care within the scope of agency services, focusing on patients requiring skilled clinical care services. This role utilizes the nursing process of assessment, planning, intervention, and evaluation in accordance with nursing practice standards, physician orders, and agency policies and procedures.

Responsibilities

• Regularly assess patients' needs and provide skilled nursing services as prescribed by the physician's plan of care.
• Initiate and adapt plans of care for assigned patients as necessary.
• Provide medication management and assess therapeutic responses.
• Coordinate patient care by evaluating outcomes and providing ongoing feedback to the care team relative to patient status.
• Collaborate with Certified Nurse Aides (CNAs) and other interdisciplinary team members.
• Notify physician of changes in patient conditions and care needs, obtaining new orders as needed.

Required Experience / Certifications / Licensure

• Minimum of 2 years of Long-Term Care (LTC) experience - Required
• Active Michigan/MILicense as confirmed by LARA and/or Nursys - Required
• Current American Heart Association BLS certification - Required
• Completion of Skills Checklist pertinent to LTC within the last year - Required
• Two supervisory reference forms completed within the last two years - Required
