LPN Full Time Position

Schedule

This position requires a schedule of 3:00 PM to 11:30 PM, 5 days a week, with rotating weekends and holiday shifts.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents, ensuring adherence to policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect and assess data regarding the resident's physical and psychosocial condition.
• Develop and implement age-appropriate and culturally relevant care plans in collaboration with the healthcare team.
• Recommend care plan revisions based on evaluations of resident outcomes.
• Participate in mentoring and orienting new staff members.
• Ensure safe delivery of care while considering cost and quality.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification (AHA or RQI only).
• A minimum of 1 year experience as an LPN, preferably in a long-term care facility.
