Job Description

A central Vermont hospice home care agency seeks an experienced Hospice RN to provide high-quality, compassionate care to patients in a home setting. The ideal candidate will possess a strong background in hospice case management and will be comfortable with rural travel.

Responsibilities

• Provide comprehensive hospice care and support to patients and families in their homes.
• Manage and coordinate patient care plans in collaboration with the hospice interdisciplinary team.
• Utilize McKesson/Netsmart EMR for documentation and patient management.
• Engage in regular patient assessments and provide health education to families.

Required Experience / Certifications / Licensure

• Prior experience in hospice case management is required.
• Experience in providing hospice care in a home setting is essential.
• Rural travel experience is preferred.
• Familiarity with McKesson/Netsmart EMR is preferred.
• Current RN licensure in the state of Vermont.
