Job Description
The Physical Therapist is responsible for providing professional rehabilitation therapy to patients in their home. This includes evaluating patients, developing and implementing individual goal-oriented therapy programs in accordance with organizational policies and procedures as well as external organizations.
Responsibilities
- Complete patient evaluation within the specified time frame and ensure holistic assessment of the client's condition.
- Develop individualized treatment programs focusing on measurable short and long-term goals.
- Prepare appropriate system documentation, including progress notes, reassessments, and discharge summaries.
- Ensure documentation is precise and submitted timely to support the need for continued skilled care.
- Coordinate service delivery according to the physician's plan of care for the client.
- Act as a supervising therapist to the Physical Therapy Assistant (PTA).
- Educate the patient and/or caregiver on a written home program.
- Participate in discharge planning throughout the patient’s course of care.
- Document case communication with other health care team members via secure messaging.
- Demonstrate flexibility with job responsibilities in all areas as assigned.
Required Experience / Certifications / Licensure
- Minimum of 1 year of PT experience required, with home care experience strongly preferred.
- Registered Physical Therapist certification required.
- BLS must be AHA certified.
- Certified Home Care experience with OASIS knowledge and current usage is required.
- EPIC documentation system knowledge is preferred.
