Job Description

We are seeking a dedicated Physical Therapist for home health services in Franklin, NC. The ideal candidate will provide comprehensive assessments, evaluations, and treatment planning for patients in their own homes, including geriatric patients and those in post-acute care following THA/TKA surgeries.

Responsibilities

• Conduct comprehensive assessments and evaluations.
• Develop and implement treatment and care plans.
• Oversee Physical Therapist Assistants (PTAs).

Required Experience / Certifications / Licensure

• Must have experience completing OASIS assessments.
• Home Health experience is mandatory.
• Current NC license to practice Physical Therapy.
• BLS/CPR certification required.
• Knowledge of PDGM payment model is a plus.
