Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care for assigned residents, ensuring that care is appropriate to the age and specific needs of the patients. The LPN will work under the supervision of a Registered Nurse (RN) and maintain collaborative relationships with nursing staff, physicians, and ancillary staff. Continuous education and maintaining clinical expertise are expected responsibilities.

Responsibilities

• Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and Interdisciplinary Team.
• Contribute to the creation of culturally relevant care plans reflecting nursing diagnoses and interventions to achieve desired outcomes.
• Recommend revisions to the plan of care after evaluation of resident outcomes and indicate the need for referrals.
• Maintain current knowledge and competency in nursing practice and participate in the mentorship/orientation of staff.
• Demonstrate leadership under the direction of the RN in the professional practice setting.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS (AHA or RQI required)
• Minimum of 1 year of experience as an LPN, preferably in a Long-Term Care (LTC) facility
