Job Description

The Case Manager RN will provide discharge planning and coordinate care for patients within an acute care setting. The role requires proficiency in Epic and experience in acute care environments. The position includes responsibilities related to patient needs assessment, care coordination, and planning for durable medical equipment (DME) as part of discharge planning.

Responsibilities

• Conduct needs assessments and create individualized care plans
• Coordinate with interdisciplinary teams to ensure patient needs are met
• Manage discharge planning processes effectively
• Provide patient education and develop plans of care in compliance with CMS guidelines

Required Experience / Certifications / Licensure

• Active Alaska RN license
• BLS certification required
• ACM certification preferred
• Minimum of 1 year of relevant nursing experience
• Epic experience required
• Acute care experience is essential
