Schedule
Variable shifts of either 8:30 AM to 4:30 PM or four 10-hour shifts from 7:30 AM to 5:00 PM. No call required; weekend and holiday rotation is necessary.
Job Description
We are seeking two experienced RN Care Managers to coordinate concurrent utilization management activities within the hospital environment. The RN Care Manager serves as the primary liaison between patients, healthcare teams, organizations, and payors during hospitalization.
Responsibilities
- Conduct comprehensive assessments of patient needs and identify resources necessary for a smooth transition to the next care setting.
- Collaborate with and lead an interdisciplinary team during daily care rounds, conferences, and planning sessions.
- Utilize excellent communication skills across various platforms including written, verbal, and electronic communications.
- Engage with community organizations to secure medical equipment, insurance authorizations, and arrange transportation for patients.
- Participate in managing the length of stay for inpatients.
- Demonstrate knowledge of hospital reimbursement methods and quality measures pertinent to care management.
Required Experience / Certifications / Licensure
- Associate or Bachelor degree-prepared Registered Nurse with 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or prior utilization review experience.
- Experience in utilization review and discharge planning is essential.
- Certification from a nationally recognized case management or utilization review organization is preferred.
- Active Basic Life Support (BLS) certification is required; CCM or ACM certification is preferred.
