Schedule

Candidate must be available to start on a Tuesday or Thursday, full shift. Minimum of 24 hours per 4 weeks with 2 weekend shifts required.

Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care for assigned residents, following defined policies and procedures under the supervision of a Registered Nurse. The LPN must demonstrate knowledge of growth and development principles relevant to the patients served, primarily focusing on adult and geriatric populations.

Responsibilities

• Collect data regarding residents' physical and psychosocial conditions on an ongoing basis, and communicate findings to the Registered Nurse and interdisciplinary team members.
• Contribute to the development of culturally relevant care plans reflecting nursing diagnoses and interventions to achieve desired outcomes.
• Recommend revisions to care plans based on evaluation of resident outcomes and collaborate with residents/families and the healthcare team.
• Maintain current knowledge and competency in Nursing Practice, participating in the mentoring and orientation of staff.
• Under the direction of the RN, provide leadership in the professional practice setting.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification
• 1 year of experience as a Licensed Practical Nurse
