Licensed Practical Nurse (LPN) - Long-Term Care

Days - 6:00 am to 6:30 pm

Job Description

We are seeking a motivated and experienced LPN to join our long-term care team. Successful candidates will provide high-quality care to our residents, supporting their daily health needs and overall well-being.

Responsibilities

• Provide nursing care to long-term patients
• Administer medications and treatments
• Monitor and document patient health status
• Collaborate with healthcare team to develop and implement care plans

Required Experience / Certifications / Licensure

• Must have a current, unrestricted Colorado LPN license
• Experience in long-term care is required
• MDS experience is preferred
