Job Description

We are seeking an experienced Home Health Physical Therapist to join our team. The ideal candidate will be responsible for delivering exceptional, personalized care to patients in their homes.

Responsibilities

• Manage a caseload for PT and PTA in the designated area.
• Conduct Home Health admissions, recertifications, and functional assessments.
• Supervise PTA and manage all patient physical therapy needs, ensuring effective communication with the physician's office.
• Complete a minimum of 5-6 patient visits per day.
• Be available for one weekend on-call every 4-6 weeks for visits, evaluations, or admissions.

Required Experience / Certifications / Licensure

• Minimum of 2 years of clinical experience in Home Health.
• Active BLS certification is required.
• Home Health experience is mandatory.
