Job Description

The Case Management Manager serves as the Team Lead for the Case Management Department, directing the daily functions of the Case Management staff. This position is responsible for ensuring that all functions of Case Management occur regularly and are documented appropriately, including communication with third-party payers and financial services.

As the second layer of review, this role addresses discrepancies in the levels of care identified by Case Management staff and escalates unresolved issues to the Chief Nursing Officer (CNO) as needed. The Case Management Manager will oversee the case management department, ensuring effective patient care coordination, resource utilization, and discharge planning. This position requires an RN license.

Responsibilities

• Direct daily functions of the Case Management staff.
• Provide input into job evaluations and work assignments.
• Ensure communication with third-party payers and financial services is documented and occurs regularly.
• Address discrepancies in levels of care and escalate issues to the CNO when needed.
• Oversee case management department operations to enhance patient care coordination and discharge planning.
• Collaborate with medical staff, insurance providers, and community resources to optimize patient outcomes while controlling costs.

Required Experience / Certifications / Licensure

Must hold a current RN license.
