Home Health Physical Therapist Assistant - Salem, OR

Location

Salem, OR and surrounding areas including South Salem, West Salem, Corvallis, Albany, Sweet Home, and Lebanon.

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 8 hours per day from 08:00 AM to 04:30 PM. Note that this facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Physical Therapist Assistant (PTA) to provide high-quality care to patients in their homes. The PTA will work closely with patients to support their rehabilitation goals and improve their overall quality of life.

Responsibilities

• Provide rehabilitation services to patients in home health settings.
• Assist patients in performing therapeutic exercises and daily activities.
• Document patient progress and communicate effectively with the healthcare team.
• Maintain compliance with patient care standards and regulations.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Basic Life Support (BCLS) certification is required.
• Reliable transportation is expected for patient visits.
• COVID vaccination series and one booster required.
