Case Manager - Day Shift

Job Description

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation.

Responsibilities

• Identify patient problems affecting treatment participation and assist in utilizing hospital resources.
• Collaborate with the interdisciplinary team to direct a clinical plan of care and facilitate safe discharge plans.
• Ensure accurate documentation and compliance with safety policies.
• Conduct team conferences and submit professional reports.
• Review and manage medical necessity as mandated by the Quality Improvement Organization (QIO).
• Maintain community resource knowledge and assist families in accessing appropriate services.
• Optimize outcomes through effective management of utilization reviews and unnecessary services.

Required Experience / Certifications / Licensure

• Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred).
• Working knowledge of the insurance industry and government reimbursement.
• Two years of experience in Case Management and Discharge Planning in an Acute Care setting.
