Job Description

The Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans as per patients' needs and physician direction. Responsible for delegating and directing all discipline-specific therapy services for assigned patients. Ensures the design of functional treatment programs that allow for effective return to community activities. Serves as an educator for patients, families, students, coworkers, and self while performing other professional duties related to patient care, development of therapy services, and program delivery.

Responsibilities

• Completes comprehensive evaluations of assigned patients and utilizes clinical reasoning to develop appropriate treatment plans and goals.
• Collaborates with the interdisciplinary team to address discharge barriers, including equipment trialing and family training.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Documents medical records clearly, accurately, and timely in compliance with guidelines and hospital policies.
• Participates in student programs, providing supervision and feedback to students.
• Actively pursues professional development and adheres to discipline-specific state practice acts and regulations.
• Implements organizational performance improvement initiatives and offers feedback for improving quality of services.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in a relevant field.
• Inpatient rehab or neuro experience preferred.
• Valid, current state license and BLS certification required prior to the first shift.
