Job Description

We are seeking an experienced healthcare professional to provide in-person care. The ideal candidate will have a strong background in acute hospital settings with a solid understanding of patient care coordination and charting, specifically using Epic systems. This position requires business casual attire and demands a high level of professionalism and teamwork.

Responsibilities

• Conduct thorough patient assessments and coordinate care plans.
• Utilize Epic charting systems for efficient patient management.
• Engage in admission and discharge planning processes.
• Collaborate with interdisciplinary teams to ensure quality care delivery.
• Monitor and document patient progress and adjust care strategies as needed.

Required Experience / Certifications / Licensure

• Must have at least 3 years of experience in an acute hospital environment.
• BLS certification required.
• Experience with patient care coordination and familiarity with CMS guidelines preferred.
• Must possess a valid MD or Compact License at the time of submission.
• Ability to float within the hospital with no restrictions or special requests.
