Job Description

Join a critical access hospital that excels in outpatient, inpatient, and skilled nursing facility care. The therapy department provides services across a variety of specialties, including wound care, lymphedema, pelvic floor, and pediatrics.

Responsibilities

• Provide quality physical therapy services to patients of all ages and diagnoses.
• Assess patient needs and develop individualized treatment plans.
• Collaborate with a multi-disciplinary team to ensure comprehensive patient care.
• Document patient progress and adjust treatment plans as necessary.
• Engage in patient and family education regarding rehabilitation and recovery.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in a physical therapy setting.
• BLS certification required.
• Active state license to practice as a Physical Therapist.
• Experience with Epic charting system preferred.
