Job Description

The Physical Therapist provides rehabilitative services to home health and hospice patients, focusing on treatment, education, and safety for individuals and their families. The therapist collaborates with healthcare teams to appraise health needs and coordinate therapy services under medical direction.

Responsibilities

• Evaluate, plan, implement, and document patient care.
• Consult with teams and refer families to appropriate personnel or community services.
• Conduct treatment and preventive procedures following medical directions.
• Supervise and educate patients and their families on rehabilitation and safety measures.

Required Experience / Certifications / Licensure

• Licensed Physical Therapist in North Dakota.
• Minimum of 2 years of experience as a Physical Therapist preferred.
• Valid driver's license and a car available for travel to patients' homes.
• Recent travel experience preferred.
• Cerner experience strongly preferred.
