Licensed Practical Nurse (LPN) - Full Time Day/Nights

All contracts must run through the holiday season. Start dates are flexible and will begin on a Tuesday or Thursday.

Job Description

The LPN will demonstrate the knowledge and skills necessary to provide age-appropriate care to patients in the assigned unit. The individual will assess data reflective of the patient's status, interpreting information necessary to identify each patient's requirements related to their specific needs to provide optimal care, particularly to adult and geriatric patient populations.

Responsibilities

• Collect relevant data pertaining to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the RN and interdisciplinary team members.
• Contribute to the development of a culturally relevant plan of care, involving residents and families in the process while promoting a healthy, safe environment.
• Recommend revisions to care plans based on evaluation of resident outcomes and indicate the need for referrals to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice; participate in mentoring and orientation activities for staff.
• Provide leadership under the direction of the RN, considering factors related to safety, cost, and the impact in the delivery of care.

Required Experience / Certifications / Licensure

• Current New York LPN licensure
• BLS certification (AHA or RQI specifically)
• A minimum of 1 year of experience as an LPN, preferably in a long-term care facility
