Licensed Practical Nurse (LPN) - Full Time

Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The role requires the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.

Responsibilities

• Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.
• Collaborate with the resident/family and members of the Interdisciplinary Team to ensure all aspects of the resident's care have been addressed.
• Participate in the mentoring/orientation of staff while maintaining current knowledge and competency in Nursing Practice.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS Certification (AHA or RQI only)
• 1 year of experience as an LPN in a Long-Term Care (LTC) facility

Schedule

The position requires full-time availability with 5x8 shifts. Flexibility to work Day, Evening, Night, and Weekend rotations is necessary, particularly during the holiday season.

Assignment Duration

This is a 13-week contract position.
