Schedule
Contracts must run through the holiday season and will start on a Tuesday or Thursday, with a full shift requirement. A minimum of 24 hours per 4 weeks, including 2 weekend shifts, is required.
Job Description
The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents in a long-term care facility. The LPN must demonstrate the knowledge and skills necessary for providing care suitable to the age-specific needs of patients, particularly adults and geriatric residents. This role entails adhering to defined policies and procedures under the supervision of a Registered Nurse and maintaining collaborative relationships with other team members.
Responsibilities
- Collect ongoing data related to residents' physical and psychosocial conditions, and communicate findings to Registered Nurses and the Interdisciplinary Team.
- Contribute to creating culturally relevant and developmentally appropriate care plans reflecting nursing diagnoses and interventions aimed at achieving desired outcomes.
- Recommend revisions to care plans after evaluating resident outcomes and determine the need for referrals to address all aspects of care.
- Maintain current knowledge and competency in nursing practice and participate in the mentoring or orientation of new staff.
- Consider factors related to safety, cost, and impact during care delivery planning.
- Demonstrate competency and knowledge in the professional practice setting while ensuring an appropriate level of clinical expertise through continuing education and in-service programs.
Required Experience / Certifications / Licensure
- Current New York Licensure as a Licensed Practical Nurse (LPN)
- BLS certification
- At least 1 year of experience as an LPN in a long-term care facility
