LPN Full Time Nights - Flexible Holiday Season

Full-time nights, working 5 shifts of 8 hours per week, including every other weekend and holidays.

Job Description

The LPN will provide comprehensive nursing care appropriate to the age of patients in their assigned unit, specifically focusing on adult and geriatric populations. This role involves both direct and indirect patient care under the supervision of a Registered Nurse, while supporting a collaborative environment with nursing staff, physicians, and ancillary personnel. The LPN is also expected to maintain clinical competence through ongoing education and participate in mentoring new staff.

Responsibilities

• Collect and assess ongoing data pertinent to residents' physical and psychosocial conditions; communicate findings to the RN and Interdisciplinary Team.
• Contribute to the development of culturally relevant care plans that address nursing diagnoses and interventions for desired outcomes.
• Propose revisions to resident care plans based on evaluated outcomes and suggest necessary referrals to ensure comprehensive care.
• Participate in mentoring and orientation of new staff; maintain knowledge of current nursing practices.
• Provide leadership in a professional setting, considering safety, costs, and impacts in care planning.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification (through AHA or RQI).
• Minimum of 1 year of experience as an LPN, preferably in a Long-Term Care facility.
