Physical Therapist - Skilled Nursing Facility

Assignment Duration

This is a 13-week contract position.

Schedule

Variable - 7 days per week departmental coverage with a generally scheduled 5-day work week, including one weekend day.

Job Description

Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) by performing evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice. Collaborate with the supervising therapist, physician, and interdisciplinary team to develop and implement individualized treatment plans.

Responsibilities

• Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation of progress, changes in plan of care, and discharges according to facility, state, federal, and payer requirements.
• Supervise therapy assistants as applicable to the role.
• Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations.

Required Experience / Certifications / Licensure

• Minimum of two (2) years of experience in the SNF/ICF setting preferred.
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license in good standing appropriate to the discipline.
