Day Shift: 5x8-Hour (08:00 - 17:00), Monday to Friday

Job Description

A Physical Therapist is needed for home health coverage in Princes County and Charles County. The candidate will provide care and therapy to adults while adhering to all protocols and delivering quality patient care.

Responsibilities

• Conduct evaluations and develop individual treatment plans for home health patients.
• Administer therapy sessions to help patients improve mobility and manage pain.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Collaborate with interdisciplinary teams to provide comprehensive patient care.
• Manage a caseload of 6 visits per day, ensuring quality care delivery.

Required Experience / Certifications / Licensure

• Maryland Physical Therapist state license required.
• Basic Life Support (BLS) certification required.
• Minimum of 1 year of home health experience preferred.
• Experience with HCHB charting system preferred.
