Job Description

We are seeking a dedicated Home Health Physical Therapist (PT) for the West Portland area and surrounding regions. The position involves providing direct patient care to individuals in their homes, ensuring they receive the highest quality rehabilitation services.

Responsibilities

• Conduct evaluations and create individualized treatment plans for patients.
• Deliver therapy interventions and assess patient progress.
• Manage up to 6 patient visits per day, with a focus on effective time management.
• Utilize the EPIC charting system and Rover application for documentation and scheduling.
• Collaborate with interdisciplinary teams to enhance patient outcomes.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• BCLS certification is mandatory.
• Prior experience with OASIS documentation is preferred.
• Must possess a valid state license for physical therapy.
• Reliable transportation is necessary for travel between patient locations.
