Acute Care Manager - Travel Position

This is a 13-week contract position.

Schedule

Day shift, 5x8 hours (08:00 - 16:30). Weekend requirement: typically one full weekend per 4 weeks, subject to department need.

Job Description

The Acute Care Manager will be responsible for care coordination and discharge planning for adult and geriatric patients in an acute hospital setting. This position requires travel and experience in patient assessments, care management, and interdisciplinary collaboration.

Responsibilities

• Coordinate patient care and discharge planning.
• Assess patient needs and determine medical necessity per evidence-based guidelines.
• Document care coordination activities using computerized systems (experience with Epic is required).
• Manage patient caseload of up to 25 individuals daily.
• Work collaboratively with interdisciplinary teams to ensure optimal patient outcomes.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute medical care management experience.
• Experience with acute hospital management and care coordination.
• Registered Nurse (RN) license required.
• Ability to float between facilities within the state.
• Commitment to work 3 of the following holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day.
• Two references covering at least one year required during the compliance process.
