Schedule

Day shift, 5x8-hour schedule (07:30 - 16:00) with weekend requirements (every other weekend or 4 weekend days in a 4-week schedule).

Job Description

The Case Manager RN will be responsible for care coordination, discharge planning, and ensuring medical necessity per evidence-based guidelines in a fast-paced acute hospital environment. The ideal candidate will have Epic experience and a strong background in acute care.

Responsibilities

• Oversee patient care coordination and discharge planning.
• Conduct continued stay reviews and pre-certification processes.
• Utilize Epic for charting and maintaining patient records.
• Collaborate with interdisciplinary teams to facilitate patient care and services.

Required Experience / Certifications / Licensure

• Current AK RN license in hand.
• BLS certification required (ACM certification preferred).
• Minimum of 1 year of experience in case management or acute care setting.
• Epic experience is a must.
