Job Description

The Physical Therapist is responsible for providing direct patient care including evaluations, consultations, and treatments to assist patients in achieving maximum physical performance. This role involves performing individualized evaluations and designing customized, goal-directed plans to improve patients' physical function and mobility.

Responsibilities

• Provide direct patient care through assessments and therapeutic interventions.
• Develop and implement individualized treatment plans tailored to patient needs.
• Monitor and document patient progress and make necessary adjustments to treatment plans.
• Educate patients and families on treatment procedures and home exercise programs.
• Collaborate with interdisciplinary teams to optimize patient care.

Required Experience / Certifications / Licensure

• Valid Physical Therapist license in the state of Illinois.
• Relevant clinical experience in the field of physical therapy.
