Day shift, 5x8-hour schedule (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a skilled Home Health Occupational Therapist (OT) to provide high-quality therapy services to patients in their homes. The ideal candidate will be responsible for assessing patients' needs, developing treatment plans, and implementing therapeutic interventions to enhance their quality of life.

Responsibilities

• Conduct comprehensive evaluations and assessments of patients’ functional abilities and needs.
• Develop and implement individualized treatment plans based on patient goals and clinical evidence.
• Assist patients in performing activities of daily living to promote independence.
• Monitor and document patients' progress and adjust treatment plans as necessary.
• Educate patients and their families on therapeutic techniques and home safety.
• Collaborate with interdisciplinary teams to deliver coordinated care.

Required Experience / Certifications / Licensure

• A minimum of 1 year of skilled home health experience is required.
• Oasis experience is mandatory.
• Must possess a current BCLS certification.
• Valid state OT license required.
