Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patient’s needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that allow for effective returns to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves, while performing other professional duties related to patient care and therapy service development.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals targeted at patient progression towards discharge.
• Complete medical record documentation accurately and timely in compliance with relevant guidelines.
• Provide supervision and a conducive learning environment for students as necessary.
• Create and pursue an individualized professional development plan with clinical management.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and engage in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state licensure for physical therapy.
