Licensed Practical Nurse (LPN) - Full Time Days/Evenings

Job Description

The Licensed Practical Nurse (LPN) provides comprehensive nursing care to residents, ensuring that age-specific needs are met in accordance with established standards and practices. The LPN will work collaboratively within an interdisciplinary team to assess, plan, implement, and evaluate individualized patient care plans.

Responsibilities

• Collect ongoing data regarding residents' physical and psychosocial conditions, communicating results to the RN and interdisciplinary team.
• Develop culturally relevant care plans that reflect patient needs and nursing diagnoses, involving residents and families in the process.
• Evaluate resident outcomes and recommend necessary adjustments to care plans, ensuring comprehensive care is provided.
• Maintain current nursing knowledge through continuing education and training, and participate in staff mentoring and orientation.
• Lead nursing practices under the supervision of the RN, ensuring high standards and safety in care delivery.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure
• Basic Life Support (BLS) certification (American Heart Association or RQI)
• A minimum of 1 year of experience as an LPN in a long-term care facility
• Availability to work evenings and weekends, including holidays

Schedule

Full-time schedule with a rotating shift, including 5 days of 8-hour shifts.
