Registered Nurse - Case Management

Job Description

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction and to promote continuity of care and cost-effectiveness through the functions of case management, utilization review, and discharge planning.

Responsibilities

• Conduct comprehensive assessments and evaluations for individual patients.
• Plan and implement patient care plans that meet individual needs.
• Coordinate care with interdisciplinary teams to ensure continuity and quality of care.
• Manage discharge planning processes effectively.
• Utilize clinical data and guidelines to promote cost-effective care.

Required Experience / Certifications / Licensure

• Minimum of 4 years of nursing experience, with at least 2 years in a healthcare setting as a Registered Nurse.
• Experience with EPIC charting system is required.
• Must be licensed to practice as a Registered Nurse in the state of North Carolina.
• BLS certification required; ACM/CCM certification preferred.
• Acute hospital experience is required.
