Outpatient Physical Therapist

Job Description

This is a 13-week contract position focused on providing quality outpatient therapy services. The Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans according to the patients' needs, goals, and physician direction. Responsibilities include the delegation and direction of all discipline-specific therapy services, ensuring the design of functional treatment programs to facilitate effective community reintegration.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge.
• Document medical records in a clear, concise, accurate, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approvals.
• Participate in the student program by offering a positive learning environment for students with appropriate supervision.
• Create an individualized professional development plan and actively pursue continuing education.
• Maintain professional licensure in good standing and adhere to ethical standards.
• Engage in organizational performance improvement initiatives.

Schedule

5x8.0 shift; evaluations last 60 minutes with follow-up appointments every 30 minutes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• Maintains licensure in good standing and completes all state-mandated continuing education requirements.
