Licensed Practical Nurse (LPN)

Job Description

The primary functions of an LPN involve assisting with assessment, planning, implementing, and evaluating the care of all patients, regardless of age, during the assigned shift. This role is responsible for meeting the established standards of skilled nursing practice, collaborating with the care team, and advocating for patients. The LPN effectively provides person-centered care and services, administers medications and treatments under the supervision of a Registered Professional Nurse, and ensures compliance with facility policy.

Responsibilities

• Take and record vital signs such as respiration, temperature, blood pressure, and heart rate.
• Report changes in the patient's/resident's conditions and maintain clear communication with other nursing personnel and responsible RN.
• Practice ethically and professionally within the scope of practice as an LPN.
• Coordinate care with the interdisciplinary team and maintain clinical competence specific to the care area.
• Document thoroughly and appropriately in the EMR/Point Click Care system.
• Participate in interdisciplinary care and collaborate with the care team.
• Respond to emergency situations and perform necessary interventions as needed.

Schedule

This is a 24-hour position covering three evenings per week, from 3:00 PM to 11:30 PM. Every other weekend and holiday availability is required, with the potential to float to other units based on staffing needs.

Required Experience / Certifications / Licensure

• Active LPN license in the state of practice.
• Preferred experience with Point Click Care.
• Completion of required health screenings and vaccinations, including MMR, Hepatitis B, TB Test, and more.
