Acute Care Case Manager - Day Shift

Job Description

We are seeking an experienced Acute Care Case Manager to join our team. The ideal candidate will demonstrate proficiency in care coordination, discharge planning, and patient assessments in an acute hospital setting.

Responsibilities

• Coordinate care and conduct discharge planning for a caseload of 20-22 patients.
• Utilize InterQual criteria and Epic charting system for admission reviews.
• Communicate effectively with interdisciplinary teams, including social services.
• Conduct needs assessments and manage prior authorizations for patient care.
• Support patients and their families throughout the care continuum, ensuring eligibility for benefits.

Required Experience / Certifications / Licensure

• Minimum of 2 years of current acute care hospital experience as a case manager.
• Knowledge of Interqual and Epic systems is essential.
• Active California state license required; pending licenses accepted.
• Must commit to at least 2 weekend shifts in a 4-week schedule.
• Experience with HMOs and IPAs is preferred.
