LPN - Full Time Weekend Nights

Schedule

This position requires working full-time nights, specifically from 11:00 PM to 7:30 AM, and includes every weekend. There may also be a potential requirement for holiday shifts.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing both direct and indirect care to residents, focusing on the individual needs of adult and geriatric patient groups. The LPN works under the supervision of a Registered Nurse and collaborates closely with other nursing staff, physicians, and ancillary teams to ensure the delivery of high-quality care.

Responsibilities

• Collect and assess data regarding residents' physical and psychosocial conditions.
• Develop and implement age-appropriate and culturally relevant care plans, collaborating with residents and families.
• Monitor and evaluate resident outcomes, suggesting revisions to care plans as necessary.
• Maintain up-to-date knowledge and clinical expertise through ongoing education and training.
• Lead in the professional practice setting under the guidance of the Registered Nurse.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure.
• Basic Life Support (BLS) certification (AHA or RQI).
• A minimum of 1 year of experience in a Long-Term Care facility.
