Case Manager

Schedule

This is a full-time day shift position with a schedule of 5x8 hours, from 08:00 AM to 04:30 PM. Weekend coverage is required — 2 weekends every 6 weeks.

This is a 13-week contract position.

Job Description

The Case Manager will be responsible for coordinating patient care by assessing needs, planning, and facilitating appropriate services. This role requires effective communication with patients, families, and multidisciplinary teams to ensure optimal outcomes and compliance with standards.

Responsibilities

• Conduct assessments and determine medical necessity based on evidence-based guidelines.
• Coordinate care and discharge planning for patients across various settings.
• Engage in family meetings and collaborate on the treatment plan.
• Utilize interdisciplinary team support for comprehensive patient care.
• Manage prior authorizations and ensure adherence to HIPAA guidelines.

Required Experience / Certifications / Licensure

• Minimum of 2 years of case management experience in an acute hospital setting.
• Experience with Epic charting system is required.
• Current licensure in the relevant field is mandatory.
• Charge RN experience is preferred.
• Background in handling patients with various diagnoses and care coordination is ideal.
