5 x 8-hour shifts per week, Monday to Friday.

Job Description

We are seeking an experienced Home Hospice RN to provide high-quality patient care in the home setting. The role involves managing patient visits within Imperial County and may require assistance in nearby territories as needed. The nurse will play a vital role in ensuring patients receive compassionate and comprehensive care in their homes.

Responsibilities

• Conduct patient visits, provide assessments, and deliver personalized care plans.
• Manage medication administration and documentation to ensure compliance and patient safety.
• Collaborate with interdisciplinary teams and communicate effectively with patients and families.
• Meet productivity goals of 24-30 visits per week during the designated hours.
• Adhere to all safety and healthcare regulations while performing duties.

Required Experience / Certifications / Licensure

• Active RN license.
• Previous hospice experience is required.
• Experience with HomeCare HomeBase (HCHB) is preferred, but training may be available for applicants without it.
• Driver's license and auto insurance are mandatory.
