Job Description

CVHCare is seeking a qualified Physical Therapist to provide services in assigned territories. This position includes patient evaluations, treatment planning, and therapy interventions in various locations, including Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington.

Responsibilities

• Conduct evaluations and assessments of patients.
• Develop individualized treatment plans based on patient needs.
• Administer therapy and rehabilitation interventions.
• Document patient progress and modify treatment plans as necessary.
• Coordinate care with other healthcare professionals as needed.

Required Experience / Certifications / Licensure

• Current California Physical Therapist License.
• BLS Certification.
• Valid Driver's License and proof of auto insurance.
• Annual TB test, MMR, Varicella, HepB, TDap (10 years), Flu vaccination, and Drug screen.
• Physical examination.
