Case Manager RN

Job Description

The Case Manager RN is responsible for overseeing patient care coordination, discharge planning, and ensuring compliance with regulatory standards in an acute care setting. The role demands effective communication and collaboration with interdisciplinary teams to enhance patient outcomes.

Responsibilities

• Conduct assessments and develop individualized care plans for patients.
• Coordinate with various departments to facilitate timely patient discharge.
• Ensure adherence to CMS guidelines and HIPAA regulations.
• Utilize Epic EMR for documentation and patient management.
• Participate in interdisciplinary team meetings and case reviews.

Required Experience / Certifications / Licensure

• Active Registered Nurse (RN) license in Alaska.
• 1 year of relevant case management experience in an acute care setting.
• Basic Life Support (BLS) certification is required; ACM certification is preferred.
• Experience with Epic EMR is required.

Schedule

This position follows a 5x8-hour day shift from 07:30 to 16:00, with a requirement to work every other weekend. Hours may slightly vary.
