Schedule

5 days per week, 8 AM - 4 PM, with on-call duties required for 1 weekend per month. Float duties to different counties as needed.

Job Description

The Registered Nurse (RN) will provide compassionate and competent care in a home hospice setting. Responsibilities include performing patient assessments, developing care plans, and ensuring the effective utilization of services while communicating with interdisciplinary teams.

Responsibilities

• Perform patient assessments and determine the appropriateness for hospice care.
• Develop and implement patient and family care plans.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
• Complete discharge assessments and maintain thorough documentation.
• Utilize age-appropriate teaching methods and document patient education tools.
• Assume case management responsibilities and prioritize patient care needs.
• Ensure compliance with hospice regulations and demonstrate knowledge of pharmacology.
• Participate in performance improvement activities and assist in orienting new employees.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience as an RN in a home hospice environment.
• Current New Jersey RN License.
• Valid New Jersey driver’s license.
• BLS certification required.
• Experience using an EMR, with strong critical thinking and communication skills.
