Integrated Case Manager for Population Health
Job Description
The Integrated Case Manager for Population Health is an essential member of the patient-centered care team responsible for providing assessment, care coordination, and advocacy services to meet the comprehensive health care needs of patients and their families. This role emphasizes collaboration and communication to promote patient safety, quality of care, and cost-effective outcomes, particularly for patients who have experienced critical events or complex health management needs.
Responsibilities
- Conduct comprehensive assessments of patients and families to gauge biomedical, psychological, social, and functional needs.
- Develop personalized patient-centered care plans to enhance care experiences.
- Engage patients and families in advocacy, ongoing communication, and health education.
- Utilize professional judgment and critical thinking to help patients overcome barriers.
- Provide counseling related to treatment decisions and end-of-life issues.
- Ensure seamless and safe transitions between care settings.
- Advocate for appropriate service delivery within health plan structures.
- Collaborate with care team members to manage patients with complex medical and social needs.
- Evaluate patient responses and modify care plans as necessary.
- Participate in process improvement activities tailored to reduce risk and improve outcomes.
Required Experience / Certifications / Licensure
- Minimum of 5 years of case management experience, including 2+ years of recent acute inpatient case management experience.
- Must have a BSN or MSW degree.
- Current and valid Michigan RN license or LMSW license required.
- Experience in discharge planning is essential.
- Strong verbal communication and writing skills.
- Excellent problem-solving and analytical skills.
Schedule
This is a full-time position with a shift from 8:00 AM to 4:30 PM, including weekend rotation. This is a 13-week contract position.
