Job Description

The RN or MSW will be responsible for care coordination and discharge planning within an acute care environment. The role requires working in various areas of the hospital, including Colby and Pacific campuses, which are located less than 2 miles apart.

Responsibilities

• Coordinate patient care and discharge planning activities across the hospital.
• Float between different units as required to ensure comprehensive patient management.
• Collaborate with interdisciplinary teams to provide optimal patient care.

Required Experience / Certifications / Licensure

• Minimum 1 year of experience in acute care settings.
• Experience in discharge planning/care coordination is strongly preferred.
• BLS certification and a valid RN license in Washington are required.
• Prior experience in diverse case management settings is highly desirable.
