Job Description

We are seeking an experienced RN or MSW to join our Case Management team in a Level II Trauma Center. The ideal candidate will have strong acute care experience as well as expertise in discharge planning and care coordination. Candidates with a diverse background in case management, particularly in Med Surg and Emergency Department settings, are strongly preferred.

Responsibilities

• Provide comprehensive case management and care coordination within the hospital.
• Engage in discharge planning to ensure a smooth transition for patients.
• Float between different units within the acute care facility as required.

Required Experience / Certifications / Licensure

• Minimum of 1 year experience in acute care.
• Discharge planning and care coordination experience is essential.
• BLS certification and a valid RN Licensure are required.
• Experience with EPIC charting system is required.
