Job Description

We are seeking an experienced Registered Nurse to join our Care Coordination team. This role involves working within an acute care facility, focusing on discharge planning and care coordination across various hospital units.

Responsibilities

• Provide care coordination and discharge planning for patients across multiple units.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Assist with needs assessments and planning of care to facilitate transitions.
• Work with patients and families to develop individualized care plans.

Required Experience / Certifications / Licensure

• Minimum of 1 year of acute care experience.
• Experience in discharge planning and care coordination is essential.
• BLS certification and active RN licensure required.
• Strong preference for candidates with diverse case management experiences, including Medical-Surgical, and Emergency Department.
