Registered Nurse (RN) / Medical Social Worker (MSW) - Acute Care

This is a 13-week contract position.

Schedule

Day shift, 4x10 hour shifts from 07:30 AM to 06:00 PM. Weekends required every other weekend.

Job Description

The RN or MSW will work within a Level II Trauma Center, focusing on care coordination and discharge planning. The role involves providing support and managing patient care across various units, including both Colby and Pacific campuses, which are located less than two miles apart.

Responsibilities

• Perform care coordination and discharge planning for patients in an acute care setting.
• Float between different units as necessary, including Med Surg and Emergency Department.
• Work on interdisciplinary teams to ensure comprehensive patient care.
• Assist in the needs assessment and plan of care for patients.

Required Experience / Certifications / Licensure

• At least 1 year of acute care experience is required.
• Experience in discharge planning and care coordination is mandatory.
• Must have a valid RN license or MSW certification.
• BLS certification is required.
• Strong preference for candidates with diverse case management experience.
